For Basic FSSAI Registration:

4. Business details including name, address, and nature of business.

5. Proof of premises (e.g., rent agreement or ownership document).

For State or Central FSSAI License (For medium and large-scale food businesses):

1. Passport-sized photograph of the applicant.

2. Identity proof of the food business operator (e.g., Aadhaar card, PAN card).

3. Proof of premises (e.g., rent agreement, utility bills, or property documents).

4. List of food products to be manufactured or processed.

5. List of equipment and machinery used in production.

6. Declaration form (Form B) duly signed.

7. Partnership deed or Certificate of Incorporation (for company).

8. No Objection Certificate (NOC) from local municipality or panchayat.

9. Water test report from a recognized public health laboratory.

10. Layout plan of the processing unit.

11. Details of Directors/Partners with address proof.

12. Proof of turnover (for the last 2 years, if available).

13. Food safety management system plan or certificate (if applicable).
