3. Trademark Logo/Name

• A clear representation of the trademark, including the logo, brand name, or label in the prescribed format.

4. List of Goods/Services

• A detailed list of goods or services for which the trademark will be used, categorized according to the NICE classification.

5. Power of Attorney (if applicable)

• A signed Power of Attorney (Form TM-48) authorizing a trademark agent or attorney to file the application on behalf of the applicant.

6. Priority Document (if applicable)

• A certified copy of the priority document if claiming priority from an earlier application filed in another country.

7. Business Registration Document

• A copy of the business registration certificate (such as GST registration or MSME registration) for companies and partnerships.

8. Address Proof

• A copy of the address proof of the applicant, such as utility bills or rental agreements.

9. Statement of Use (if applicable)

• A declaration of the use of the trademark in commerce, stating when the trademark was first used.

10. Photographs (if applicable)

• Passport-sized photographs of the applicant or the authorized signatory.
